General/Group Insurance
Insurance coverage is an agreement between you and your insurance carrier. As a courtesy to our patients we will complete any necessary forms to collect payments for services rendered from your primary insurance company. Our insurance department will also file claims to your secondary insurance when a balance is due.

Although most insurance companies do provide coverage for chiropractic care, the amount your insurance company pays varies from one policy to another. As a courtesy to our patients, our staff will contact your insurance company and request benefits for your plan. We will do our best to inform you of the expected amount of coverage you will have from your insurance company.

Insurance plans usually have a deductible with a percent that the patient is responsible to pay at each office visit. We ask that patients make this payment at the time of service unless other arrangements have been made prior to your office visit.

Managed Care Insurance
We will honor all managed care contracts in which our office is participating. We will work with you, your primary care physician, and your insurance company to comply with the special procedures required by managed care insurance companies. You may be required to get a referral from your primary care physician, in which case it is the patient's responsibility to contact their PCP and request a referral for treatment in our office. If authorization for treatment is needed beyond the referral, our insurance department will contact your insurance company.

Managed care contracts vary according to the terms of your policy. You will be expected to make your co-payments at the time of service. We will accept contracted discounts if we are in your managed care group. If your managed care company denies your treatment, then you will be responsible for services rendered. We will file secondary insurance when a balance is due.

Workers Compensation - "On The Job Injury"
A work related injury must be filed with your employer to open a workers compensation claim. Our staff will contact your employer for verification of work related injury and see if a workers compensation claim has been filed on your behalf.

Most Workers Compensation Insurance Carriers operate under managed care. This means that we will have to receive authorization for treatment. If your claim is approved, your bill will be covered in full by the employers' insurance carrier. If your claim is denied, we will bill your personal health insurance with a denial from your employers' insurance company.


Auto Accident and Personal Injury
Most patients involved in an accident have insurance that will cover chiropractic care. In order for us to wait for the insurance company to pay for your injuries sustained in an accident, we require the following:


  • The name and address of your insurance company.
  • A medical claim opened and a valid claim number for use in billing.
  • The name and phone number of the insurance adjuster handling your case.
  • If an attorney has been retained, a signed letter of protection for services rendered.
  • Although you are ultimately responsible for your bill, our office will wait for settlement up to 6 months after you are released from “active care”.

Medicare
Our doctors are participating providers in the Medicare Program. This does not mean Medicare will cover all services in this office. Manipulation is usually a covered service by Medicare. Any therapy, x-rays, or exam done by the chiropractor is considered a non-covered service. However, most secondary insurances will cover services that are not covered by Medicare.

Medicaid
Our doctors are participating providers in the Medicaid Program.
